Health Record Must Be Completed by July 31, 2026
The Southern Maine Huskies website (southernmainehuskies.com) made a link available to you to get Pre-participation Physical Evaluation (PPE) instructions for intercollegiate athletics participation at USM. The link will also be available on your My Campus Portal.
The entire Pre-Participation Physical Evaluation (PPE) is online, and the process may take up to an hour to complete.  **Student-athletes in-person pre-participation physical exam will take place at the University of Southern Maine. Student-athletes will participate in their physical exam according to their team schedule. **
Gather the following information together prior to completing Health Record:
- Your Immunization Shot Records. (Click on the following link for Immunization Requirements Information
- Your Health Insurance Enrollment Card (both sides of card must be uploaded)
- Proof of sickle cell test (for "new" USM student-athletes)
- Your USM MaineStreet ID # (7 digits)
- You MUST use your @maine.edu email!
Important information that you need to know and share with your folks at homeÂ
A) THE PPE IS REQUIRED FOR ALL STUDENT-ATHLETES PRIOR TO PARTICIPATION IN INTERCOLLEGIATE ATHLETICS. It must be done by our medical personnel on campus. There are no exceptions to this requirement.
B) DEADLINES FOR COMPLETING THE HEALTH RECORD (a web-based electronic health history and repository for medical release, consent and waiver form) is:Â July 31, 2026Â for all sport participants
C) All first year/first-time participants as a student-athlete at USM must have an "initial" comprehensive pre-participation physical evaluation (PPE) completed at USM Health and Counseling Services (UHCS). Your primary health insurance will NOT be billed for this PPPE. This is not meant to replace your annual exam with your PCP.
D) All "returning" student-athletes (any student-athlete who has had a previous "initial" PPE) must have a medical history update and blood pressure screening at UHCS prior to participation in intercollegiate athletics each year.  Your primary insurance will NOT be billed for this exam. This is not meant to replace your annual exam with your PCP.
E) Complete the required Health History (New Athlete) or Health History Update (Returning Athletes) and all other forms listed under "athlete pre-participation forms" in your Health Record. Do this before your deadline stated above.
F) SICKLE CELL TESTING: All first year/first-time participants as a student-athlete at USM must show proof of sickle cell testing and provide results or have the testing conducted at UHCS. Test results can be uploaded under "Uploads" once in your Health Record.
- Acceptable tests include (but may not be limited to) sickle cell solubility test and hemoglobin electrophoresis.
- "Newborn screen" tests may not meet the minimum testing standard set by the NCAA. To accept a newborn screen, the results MUST show the type of testing performed and it must meet the minimum standard of a sickle cell solubility test.
- Should you opt to have the testing done at UHCS you will NOT be billed for the blood draw, but the outside lab fee (Quest Diagnostics) will be charged to your primary insurance.
G) You must have adequate health insurance coverage that includes coverage for injuries/illness incurred while participating in intercollegiate athletics in the state of Maine. Upload both sides of your health insurance card under "Uploads" in the Health Record
