Husky Student-Athletes: Be Sure to Complete Your Pre-Participation Physical 2021-2022

PPE INSTRUCTIONS FOR PROSPECTIVE STUDENT-ATHLETES AT USM

Get a jump on these medical eligibility requirements for intercollegiate athletic participation at USM!!

Beginning June 9, 2021 the Southern Maine Huskies website (southernmainehuskies.com) made a link available to you to get Pre-participation Physical Evaluation (PPE) instructions for intercollegiate athletics participation at USM.  The entire Pre-Participation Physical Evaluation (PPE) is online, and the process may take up to an hour to complete.  **Student-athletes in-person pre-participation physical exam will take place at the University of Southern Maine. Student-athletes will participate in their physical exam according to their team schedule. More information is included in the above link. **

Gather the following information together prior to completing Privit Profile:

  1. Your Immunization Shot Records. (Click on the following link for Immunization Requirements Information
  2. Your Health Insurance Enrollment Card (both sides of card must be uploaded to Privit Profile)
  3. Your USM MaineStreet ID # (7 digits)
  4. You MUST use your @maine.edu email address as your email address in Privit profile!

Important information that you need to know and share with your folks at home 

A) THE PPE IS REQUIRED FOR ALL STUDENT-ATHLETES PRIOR TO PARTICIPATION IN INTERCOLLEGIATE ATHLETICS. NO EXCEPTIONS

B) DEADLINES FOR COMPLETING PRIVIT PROFILE (a web-based electronic health history and repository for medical release, consent and waiver form) ARE: 

  • July 31, 2021 for all fall sports participants
  • August 9, 2021 for all winter and spring sports participants

C) All first year/first-time participants as a student-athlete at USM must have an "initial" comprehensive pre-participation physical evaluation (PPE) completed at USM Health and Counseling Services (UHCS). Your primary health insurance will be billed for this PPE – Do not have a physical by your own PCP because your health insurance probably will not pay for a second one in the same calendar year.

D) All "returning" student-athletes (any student-athlete who has had a previous "initial" PPE) must have a medical history update and blood pressure screening at UHCS prior to participation in intercollegiate athletics each year.  Your primary insurance will only be billed for a medical history of bodily systems and a blood pressure check, not an annual physical.

E) Complete the Privit Profile (step 2), prior to the PPE at UHCS. Do this before your deadline (July 31 [fall sports], August 9 [winter & spring sports])

F) You must have adequate health insurance coverage that includes coverage for injuries/illness incurred while participating in intercollegiate athletics.  Upload photo images of both sides of your health insurance card under "Manage Documents" in Privit Profile in Step #2 below. (The simplest way is to take phone images of both sides of your insurance card on your smart phone, log-in to Privit Profile from your web browser of your phone and upload each image into "Manage Documents")

G) For new athletes to USM, first-years or transfers, you need to complete IMPACT concussion baseline test on line.

It will take about 20 minutes to complete. Please make sure you take it in a quiet area free from interruption. Make sure you do not get distracted by other people, your cell phone, etc. If possible, it is best taken with the use of an external mouse.
